کارآمدی درمان مبتنی بر پذیرش و تعهدبر اضطراب و افسردگی زنان باردار به وسیله لقاح خارج از رحمی
درمان مبتنی بر پذیرش و تعهد، یکی از درمانهای نسل سوم است که در آن تلاش میشود بهجای تغییر شناختها، ارتباط روانشناختی فرد با افکار و احساساتش افزایش یابد. هدف پژوهش حاضر تعیین کارآمدی درمان مبتنی بر پذیرش و تعهد بر اضطراب و افسردگی در زنان بارداری است که نازایی داشته و بهوسیله درمان لقاح خارج رحمی باردار شدهاند. در این پژوهش از طرح شبه آزمایشی از نوع پیشآزمون- پسآزمون، پیگیری با گروه کنترل استفاده شد. نمونه پژوهش به شیوه در دسترس از میان افراد دارای نازایی اولیه یا ثانویه که با لقاح خارج رحمی باردار شده و در دامنه سن بارداری ۱۴ تا ۲۲ هفته بودند انتخاب شدند. ۳۵ نفر خانم باردار با ویژگیهای یادشده بهصورت تصادفی در دو گروه آزمایش (۱۷ نفر) و شاهد (۱۸ نفر) جایگزین شدند. ابزار پژوهششامل مصاحبه ساختاریافتهSCID برای تشخیص اضطراب و افسردگی، سیاهه افسردگی بک و سیاهه اضطراب بک بود که بهصورت پیشآزمون، پسآزمون و پیگیری یکماهه پس از اتمام درمان انجام شد. گروه آزمایش در ۸ جلسه ۹۰ دقیقهای درمان مبتنی بر پذیرش و تعهد شرکت کردند ولی گروه کنترل هیچگونه آموزشی دریافت نکردند. برای بررسی فرضیهها از آزمون تحلیل واریانس مختلط بین – درون آزمودنیها (SPANOVA) استفاده شد. نتایج آزمون تحلیل واریانس مختلط نشان از وجود تفاوت معنادار اثر بین گروهی اضطراب و افسردگی در سطح معناداری کمتر از۰۵/۰ بود؛ لذا با اطمینان ۹۵ درصد میتوان نتیجه گرفت که اثربخشی مداخله مبتنی بر پذیرش و تعهد در کاهش اضطراب و افسردگی مؤثر بود. مداخله مبتنی بر پذیرش و تعهد بهواسطه افزایش انعطافپذیری روانشناختی باعث کاهش معنیدار افسردگی و اضطراب زنان باردار تحت درمان لقاح خارج رحمی در گروه آزمایش در مرحله پسآزمون و نیز دوره پی گیری یکماهه شد. به نظرمی رسد درمان مبتنی بر پذیرش و تعهد بهعنوان درمانی مکمل، در کنار درمانهای طبی ناباروری در بهبود روند بارداری و سلامت مادر و جنین میتواند نقشی بسزا داشته باشد.

کارایی درمان فراشناختی بر کاهش علائم و شدت اضطراب صفت در افراد مبتلا به اختلال اضطراب فراگیر
هدف پژوهش حاضر بررسی کارایی درمان فراشناختی بر علائم آسیبشناختی و شدت اضطراب مبتلایان به اختلال اضطراب فراگیر بود. در این مطالعه از طرح تجربی تک موردی از نوع خط پایه چندگانه استفاده شد. سه زن و دو مرد مبتلا به اختلال اضطراب فراگیر از طریق مصاحبه تشخیصی و مصاحبه بالینی ساختاریافته براساس راهنمای تشخیصی و آماری اختلالهای روانی DSM-IV برای محور یک به روش نمونهگیری هدفمند انتخاب شدند و پس از احراز شرایط لازم در این پژوهش شرکت کردند. درمان فراشناختی به مدت 8 تا 12 جلسه هفتگی به کار گرفته شد و مرحله پیگیری نیز طی 6 و 12 ماه پس از پایان درمان دنبال شد. پرسشنامه اختلال اضطراب فراگیر (GADQ-IV)، پرسشنامه اضطراب بک (BAI) و پرسشنامه اضطراب صفت به عنوان ابزار ارزشیابی در مراحل پیش آزمون، پس آزمون و پیگیری به کار گرفته شدند. به منظور تحلیل دادهها، از روش فرمول درصد بهبودی و شاخص تغییر پایا (RCI) استفاده شد. نتایج نشان داد که اثر درمان فراشناختی بر آماجهای درمان (علائم اختلال اضطراب فراگیر و شدت اضطراب) در مبتلایان به اختلال اضطراب فراگیر معنادار است و درمان فراشناختی در کاهش علائم اختلال اضطراب فراگیر از کارایی لازم برخوردار است.

مشکلات رفتاری و درمانهای روان شناختی، اثربخشی درمان تعاملی والد-کودک بر پرخاشگری کودکان، یک کارآزمایی کنترل شده تصادفی مقدماتی
بازی درمانی شیوهای از فرایند تغییر رفتار در کودک است که بر تعامل والدین و کودک مبتنی میباشد. هدف از پژوهش حاضر تعیین اثر بخشی گروه درمانی رابطه والد-کودک بر کاهش پرخاشگری در کودکان دارای والدین تحت درمان نگهدارنده با متادون میباشد. در یک کارآزمایی بالینی با پیشآزمون، پسآزمون و گروه کنترل، در خلال آذر ماه 1393 لغایت آبان ماه 1394 ، از میان خانوادههای دو والد تنی دارای مادر تحت درمان متادون مراجعه کننده به دو کلینیک درمان متادون در تهران که دارای فرزندانی در بازه سنی 8-5 سال بودند، 85 مادر بهطور هدفمند انتخاب شدند و پس از انجام غربالگری بر اساس شاخص پرخاشگری،50 کودک که بر اساس پرسشنامه پرخاشگری شهیم و با توجه به نقطه برش مفروض بالاترین نمرات را کسب نمودند، پس از انجام مصاحبه بالینی وارد فرایند پژوهش شدند. آزمودنیها بهطور تصادفی و از طریق نرم افزار اکسل آفیس در دو گروه آزمایش و کنترل تخصیص یافتند (هر گروه 25 نفر). برنامه درمانی بر اساس مدل لندرث و براتون به مدت 10 جلسه 45 دقیقهای به گروه آزمایش ارائه شد و گروه کنترل در لیست انتظار قرار گرفت. دو هفته پیش و پس از فرایند درمان آزمودنیها توسط پرسشنامه پرخاشگری (میزان پاسخ دهی=94%) مورد ارزیابی قرار گرفتند. از فرایند جلسات درمانی رونوشت برداشته شد و دادههای کیفی به وسیله نرم افزار اطلس تی آی 5 تحلیل شدند. همچنین دادههای کمی با استفاده از آزمون کای اسکوئر و تحلیل کواریانس چند متغیره مورد تحلیل قرار گرفتند. نتایج نشان داد که بازی درمانی موجب کاهش شاخص پرخاشگری در هر سه مولفه جسمانی، کلامی و رابطه ای در کودکان میشود (001/0>p). یافتهها این تحقیق میتواند در اثرگذاری مداخلات روان شناختی به عنوان رویکردی مستقل و یا مکمل در کنار درمانهای دارویی در کودکان دارای اختلالات رفتاری و روان پزشکی حائز اهمیت و دارای کاربردهای بالینی باشد.

سیر تحولی اعتماد در نمونهای از کودکان ایرانی
با توجه به نقش تعیینکننده اعتماد در سازگاری کودکان و رفتارهای پرخطر، هدف پژوهش حاضر تعیین سیر تحولی اعتماد در نمونهای از کودکان ایرانی بود. در مطالعه مقطعی حاضر، از میان جامعه آماری کلیه دانشآموزان دبستانی شهر تهران در سال تحصیلی ۹۳-۱۳۹۲، ۲۴۰ دانشآموز دبستانی در مقاطع مختلف تحصیلی اول تا ششم ابتدایی به روش نمونهگیری خوشهای در ۴ منطقه تهران انتخاب شدند. از آزمونهای اتمام حجت و اعتماد و همچنین از آزمون سنجش خطرپذیری کودکان در موقعیتهای اجتماعی برای بررسی سیر تحولی اعتماد استفاده گردید. اطلاعات گردآوری شده توسط شاخصهای آمار توصیفی و آمار استنباطی شامل آزمونهای تحلیل واریانس یکراهه و چندراهه مورد تجزیهوتحلیل قرار گرفت. یافتهها نشان داد که با افزایش سن میزان نرخ اعتماد نیز افزایش مییابد، همچنین نتایج نشان داد که با افزایش سن با افزایش خطرپذیری روبهرو هستیم، که نمایانگر ارتباط مثبت بین اعتماد و خطرپذیری در این کودکان میباشد. با توجه به نتایج پژوهش و مشاهده سیر صعودی میزان اعتماد و خطرپذیری در کودکان، لزوم توجه هر چه بیشتر به کودکان بهعنوان جامعه آسیبپذیر در مقابل آسیبهای احتمالی و رفتارهای پرخطر مشخص میشود.

مقایسه اثربخشی درمان مبتنی بر عدم تحمل بلاتکلیفی، درمان دارویی با بوسپیرون و درمان ترکیبی در بهبود علائم بیماران مبتلا به اختلال اضطراب فراگیر
اختلال اضطراب فراگیر با توجه به سیر مزمن، شیوع زیاد و همبودی بالای با سایر اختلالهای روانشناختی، بهعنوان یکی از مهمترین اختلالهای ناتوانکننده بزرگسالان مطرح شده است و پرداختن به روشهای درمانی مؤثر و اقتصادیتر برای این اختلال سودمند و ضروری است. هدف پژوهش حاضر مقایسه اثربخشی درمان عدم تحمل بلاتکلیفی، بوسپیرون و درمان ترکیبی عدم تحمل بلاتکلیفی با بوسپیرون در بهبود علائم بیماران مبتلا به اختلال اضطراب فراگیر بود. در یک کارآزمایی بالینی با پیشآزمون و پسآزمون، از بین مراجعهکنندگان به مراکز و کلینیکهای روانشناختی و روانپزشکی شهر تهران، 24 بیمار مبتلا به اختلال اضطراب فراگیر انتخاب شدند و بهطور تصادفی در سه گروه درمان عدم تحمل بلاتکلیفی، بوسپیرون (60-5 میلیگرم در روز) و ترکیبی جایگزین شدند. دادهها در مرحله قبل و بعد از مداخله از طریق مقیاس اختلال اضطراب فراگیر (GADS-7)، مقیاس عدم تحمل بلاتکلیفی (IUS)، پرسشنامه نگرانی ایالت پنسیلوانیا (PSWQ)، پرسشنامه افسردگی بک- ویرایش دوم (BDI-II) و مقیاس سازگاری اجتماعی و شغلی (WSAS) گردآوری شدند. تحلیل دادهها با استفاده از روشهای آماری کروسکالوالیس، U منویتنی و تحلیل کوواریانس یکطرفه مورد تجزیه و تحلیل قرار گرفت. نتایج پژوهش نشان داد کههر دو درمان عدم تحمل بلاتکلیفی و ترکیبی در مقایسه با بوسپیرون منجر به کاهش علائم اختلال اضطراب فراگیر، عدم تحمل بلاتکلیفی، نگرانی، افسردگی و افزایش سازگاری اجتماعی- شغلی شدند (01/0> p). نتایج آزمون U منویتنی و تحلیل کوواریانس یکطرفه نیز نشان دادند که بین اثربخشی درمان عدم تحمل بلاتکلیفی و ترکیبی تفاوت معناداری وجود نداشت. نتایج این پژوهش در کل نشان داد که درمان عدم تحمل بلاتکلیفی و ترکیبی در بهبود علائم اختلال اضطراب فراگیر مؤثرتر از بوسپیرون (60-5 میلیگرم در روز) هستند. با این وجود، به نظر میرسد افزودن دارو (بوسپیرون) به درمان عدم تحمل بلاتکلیفی منجر به بهبودی بیشتر بیماران مبتلا به اختلال اضطراب فراگیر نمیشود.

اثربخشی درمان روایتی- سازهگرایانه به شیوه "بازگشت به آینده" بر کاهش نشانههای اختلال فشار روانی حاد ناشی از وقوع تصادف منجر به اغماء در همسر
پژوهش حاضر با هدف تعیین اثربخشی درمان روایتی- سازه گرایانه به شیوه "بازگشت به آینده" بر کاهش نشانههای اختلال فشار روانی حاد انجام شد. روش: جامعه آماری این پژوهش شامل زنان 18 تا 40 سالهای بود، که در اثر آسیب روانی ناشی از تصادف منجر به اغماء در همسرانشان، به اختلال فشار روانی حاد مبتلا شده بودند و همسرانشان در سال 1392(از ابتدای خرداد تا پایان مرداد)، در بخش مراقبتهای ویژه بیمارستان پورسینای رشت بستری بودند. از میان 42 زن، تعداد 3 زن (3n=)، با استفاده از روش نمونهگیری هدفمند و بر اساس پرسشنامه واکنش فشار روانی حاد استنفورد (2000) انتخاب شدند. در پژوهش حاضر از طرح تجربی تک موردی از نوع چند خط پایه غیرهمزمان از خلال چند آزمودنی استفاده شده است. مداخله صورت گرفته شامل به کارگیری شیوه درمانی "بازگشت به آینده" بود، که به مدت 13 جلسه 60 دقیقهای بر روی آزمودنیها اعمال شد. بطور کلی این پژوهش از سه مرحله تشکیل شده بود، که این مراحل شامل ارزیابی سطح پایه، دوره انجام مداخله و مرحله پیگیری بود، که 5/1 و 3 ماه پس از پایان درمان صورت پذیرفت. یافتهها: نتایج حاصل از فرمول ضریب تغییرپذیری کوهن حاکی از آن بود که این شیوه درمانی، موجب کاهش معنادار در نشانگان اختلال فشار روانی حاد گردید (89/1-d= ). همچنین این برنامه درمانی موجب کاهش معنادار در خرده مقیاسهای اختلال فشار روانی حاد، شامل نشانگان تجزیهای (78/1-d= )، نشانگان تجربه مجدد (82/1-d= )، نشانگان اجتنابی (08/2-d= )، و نشانگان بیشانگیختگی (88/1-d= ) گردید. نتایج این پژوهش نشان داد که شیوه درمانی "بازگشت به آینده" در درمان اختلال فشار روانی حاد زنان از کارایی لازم برخوردار است.

اثربخشی رواندرمانی مثبتنگر بر رضایت از زندگی، خوشبینی و عواطف مثبت در دانشجویان
پژوهش حاضر با هدف تعیین اثربخشی برنامه رواندرمانی مثبتنگر بر افزایش رضایت از زندگی، خوشبینی و عواطف مثبت در دانشجویان انجام شد. روش پژوهش نیمه آزمایشی با طرح پیشآزمون، پسآزمون با گروه گواه بود. 34 نفر به عنوان نمونه پژوهش از بین 1200 نفر از دانشجویان کارشناسی ورودی سال تحصیلی 93-1392 دانشگاه شهید بهشتی با استفاده از روش نمونهگیری در دسترس انتخاب و به صورت تصادفی در دو گروه آزمایشی و گواه جایگزین شدند. ابزارهای این پژوهش شامل مقیاس رضایت از زندگی (داینر و همکاران، 1985) ، فرم تجدید نظر شده آزمون جهتگیری زندگی (شییر، کارور و بریگز، 1994) و برنامه عاطفه مثبت و منفی (واتسون، کلارک و گری، 1988) بود. برنامه رواندرمانی مثبتنگر به صورت هفتگی طی شش جلسه دو ساعته به گروه آزمایشی، آموزش داده شد. نتایج تحلیل کوواریانس نشان داد برنامه رواندرمانی مثبتنگر در افزایش رضایت از زندگی، خوشبینی و عواطف مثبت در دانشجویان مؤثر بوده است (05/0p<) و نتایج بعد از دو ماه در مرحله پیگیری نیز پایدار مانده بود. در مجموع نتایج این پژوهش حاکی از آن است که برنامه رواندرمانی مثبتنگر در افزایش میزان رضایت از زندگی، خوشبینی و ایجاد و افزایش عواطف مثبت در دانشجویان، برنامهایی سودمند است و از این برنامه میتوان در جهت ارتقاء سلامت روان در دانشجویان ورودی جدید دانشگاهها استفاده کرد.

بررسی تأثیر مشاوره مبتنی بر پذیرش و تعهد (ACT) بر انطباقپذیری مسیر شغلی دانشجویان
هدف پژوهش حاضر تعیین میزان تأثیر مشاوره مبتنی بر پذیرش و تعهد بر انطباقپذیری دانشجویان بود. روش پژوهش شبه آزمایشی، با طرح پیشآزمون- پسآزمون- پیگیری با گروه گواه بود. جامعه آماری شامل دانشجویان دانشگاه اصفهان بود که 40 نفر به شیوهی نمونهگیری داوطلبانه انتخاب و با گمارش تصادفی در دو گروه آزمایش و گواه (هر گروه 20 نفر) قرار گرفتند. گروه آزمایش در 8 جلسه، بهصورت هفتگی شرکت کردند. دادهها با استفاده از پرسشنامه انطباقپذیری مسیر شغلی ( (CAASگردآوری و از طریق تحلیل واریانس با اندازههای تکراری مورد تحلیل قرار گرفتند. یافتهها تفاوت معناداری بین دو گروه در انطباقپذیری مسیر شغلی و خرده مقیاسهای آن درزمینهی دغدغه، کنجکاوی، کنترل و اعتماد در مرحله پسآزمون نشان داد. همچنین نتایج آزمون پیگیری حاکی از پایداری اثر مداخله مشاوره مبتنی بر پذیرش و تعهد بر افزایش انطباقپذیری و خرده مقیاسهای آن بود. در نهایت از مشاورهی مبتنی بر پذیرش و تعهد میتوان بهعنوان یک روش مؤثر در افزایش انطباقپذیری استفاده کرد.

مقایسه اثر بخشی درمان های تکمیلی آموزش روانی-فردی وآموزش روانی-خانواده بر علایم افسردگی و شیدایی بیماران دو قطبی نوع یک تحت درمان دارویی
درمانهای مکمل مانند آموزش روانی کانون توجه جامعهی درمانی بیماران دوقطبی و بسیاری دیگر از بیماریهای مزمن روانی و طبی قرار گرفته است. هدف پژوهش حاضر سنجش مقایسه اثر بخشی درمانهای مکمل آموزش روانی-فردی و آموزش روانی-خانواده در کنار درمان روانپزشکی بیماران دوقطبی نوع یک، درقیاس با درمان روانپزشکی تنها در پیشگیری از بروز دورههای افسردگی و مانیا بوده است. پس از وارسی معیارهای ورود و خروج یک نمونه 66 نفری از بیماران دچاراختلال دوقطبی نوع یک با شیوه هدفمند انتخاب شدند. افراد به طور تصادفی در سه گروه آموزش روانی-فردی، آموزش روانی-خانواده و گروه کنترل قرار داده شدند. سنجش علایم افسردگی و شیدایی با استفاده از آزمون های سنجش شیدایی یانگ و سنجش افسرگی همیلتون صورت گرفت. افراد در سه نوبت پیشآزمون، پسآزمون و پیگیری مورد سنجش واقع شدند. دادهها از پرسشنامهها استخراج و با استفاده از تحلیل کوواریانس چند متغیری تحلیل شد. نتایج تحلیلهای آماری در شیدایی تفاوتهای معناداری را میان گروههای تحقیق نشان داد. گروه آموزش روانی-خانواده همراه با دارو درمانی در پس آزمون به گونه معنادار میانگین پایینتری از گروه کنترل (درمان روانپزشکی تنها) از نظر علایم شیدایی داشتند، اما در پیگیری تنها میانگین گروه آموزش روانی-خانواده از گروه کنترل در علایم شیدایی پایینتر بود. اثر معنادار دیگری بدست نیامد. آموزش روانی-خانواده علایم شیدایی بیماران را در کوتاه مدت و بلند مدت کنترل میکند. در مجموع میتوان نتیجه گرفت که درمان مکمل آموزش روانی-خانواده در پیشگیری ازعود غیره منتظره علایم اپیسودیک شیدایی در بیماران دوقطبی نوع یک که تحت درمان روانپزشکی هستند، موثر است و در کنار درمان روانپزشکی آنان توصیه میشود.
